TERMS & CONDITIONS
These Terms & Conditions (“Terms”) govern your access to and use of the Revive Medical Wellness website, online services, patient-facing digital resources, membership programs, promotions, vendor links, and other services to which these Terms are expressly made applicable.
By accessing or using our website, creating an account, purchasing an eligible service or membership, or otherwise affirmatively accepting these Terms, you acknowledge that you have read, understood, and agreed to them.
If you do not agree, do not use the applicable website or service.
These Terms do not replace any informed consent, treatment consent, financial agreement, privacy notice, membership agreement, prescription documentation, or other document that Revive Medical Wellness may require in connection with medical care.
1. Medical Information; No Website-Based Medical Advice
Information provided through the Revive Medical Wellness website, social media, emails, educational materials, videos, graphics, articles, or other general communications is provided for educational and informational purposes only.
Such information does not constitute individualized medical advice, diagnosis, treatment, or a provider-patient relationship.
Medical recommendations can only be made after an appropriate evaluation by a qualified healthcare professional.
Do not disregard or delay obtaining professional medical advice because of information appearing on our website.
If you are experiencing a medical emergency, call 911 or seek emergency medical care immediately.
2. Medical Services and Treatment Results
Medical, wellness, aesthetic, regenerative, hormone-related, weight-management, injectable, skincare, hair-restoration, body-contouring, and other treatments may involve benefits, limitations, risks, side effects, contraindications, and alternatives.
Individual outcomes vary. Photographs, testimonials, examples, descriptions, and marketing materials are illustrative and do not constitute a promise, warranty, or guarantee of any particular result.
Eligibility for any treatment is determined individually by the treating provider.
Revive Medical Wellness may decline, postpone, modify, or discontinue a treatment when clinically appropriate or when treatment would not be in the patient's best interest.
3. Informed Consent
Acceptance of these Terms does not constitute informed consent for a medical procedure.
Before applicable treatments, patients may be required to complete separate medical histories, disclosures, informed-consent documents, financial agreements, medication acknowledgments, or other clinical documentation.
Where there is a conflict between these general Terms and a treatment-specific informed consent concerning clinical risks or treatment instructions, the treatment-specific document controls with respect to that treatment.
4. Prescription Medications and Compounded Products
Prescription medications are available only when prescribed by an appropriately licensed healthcare professional following an appropriate clinical evaluation.
Certain treatment programs may involve compounded medications or therapies obtained from licensed compounding pharmacies, when clinically appropriate. Revive materials also describe compounded medications and peptide therapies as part of individualized treatment protocols.
Patients must use medications only as prescribed and promptly report significant adverse reactions or concerns to the appropriate healthcare professional.
5. Appointments
Patients are responsible for providing accurate contact information and arriving on time for scheduled appointments.
Appointment availability, provider availability, treatment duration, and scheduling are subject to change.
Specific cancellation, rescheduling, late-arrival, deposit, and no-show requirements communicated during booking or contained in a separate clinic policy will apply to the applicable appointment.
6. Fees, Payments and Refunds
Prices may vary based on treatment, dosage, product, treatment area, provider recommendation, membership status, promotional eligibility, and other factors.
Unless prohibited by law, patients are responsible for charges they authorize and for understanding the financial terms presented before purchasing a service, product, package, or membership.
Any specific deposit, cancellation, package-expiration, refund, financing, membership-payment, or no-show policy presented to you at purchase or booking forms part of your transaction.
Nothing in these Terms limits rights or remedies that cannot lawfully be waived.
7. Revive Membership
Revive Medical Wellness may offer membership programs providing benefits such as member-only pricing, discounts, complimentary services or perks, special offers, preferred or priority access, early access, member events, and other benefits. These concepts are reflected in Revive's existing membership materials.
Membership benefits are subject to the specific membership level selected and the membership terms presented at enrollment.
Benefits:

have no cash value unless expressly stated otherwise;

may be subject to eligibility, scheduling, clinical suitability, exclusions, or availability;

may not be combined with other promotions unless expressly permitted;

are personal to the enrolled member unless Revive expressly permits transfer; and

do not guarantee eligibility for any medical treatment.
Clinical appropriateness always takes priority over a membership benefit.
Where recurring billing, minimum terms, cancellation notice, renewal, benefit rollover, freezes, or refund rules apply, those terms must be disclosed in the applicable membership enrollment agreement.
8. Promotions, Discounts and Complimentary Services
Promotions, discounts, complimentary services, gifts, special pricing, and limited-time offers may have separate eligibility requirements, expiration dates, exclusions, or other conditions.
Revive Medical Wellness reserves the right to correct genuine pricing or typographical errors and to discontinue or modify promotional offers prospectively, subject to applicable law and previously completed transactions.
9. Vendor Shopping Links and Third-Party Products
Revive Medical Wellness may provide patients with clinic-specific links or access to selected third-party vendors.
Revive's promotional materials describe these links as potentially providing benefits such as exclusive discounts, early access to new products, product information, special offers, and curated product selections.
When you purchase directly from a third-party vendor, your purchase may constitute a transaction between you and that vendor, not Revive Medical Wellness. The vendor's terms, privacy practices, shipping policies, warranties, return policies, and refund policies may apply.
A link, recommendation, or product discussion does not guarantee that a product is appropriate for every individual.
10. Third-Party Websites
Our website may contain links to websites or services operated by third parties.
Revive Medical Wellness does not control third-party websites and is not responsible for their availability, security, content, privacy practices, terms, products, fulfillment, or services.
You should review the applicable third party's terms and privacy policies before providing information or completing a transaction.
11. Privacy and Health Information
Revive Medical Wellness respects patient privacy and handles personal information in accordance with applicable privacy requirements and its applicable privacy notices and policies.
Do not use ordinary website forms, social-media messages, or other non-designated communication channels to transmit sensitive medical information unless Revive specifically indicates that the channel is appropriate for that purpose.
Use of the website does not by itself establish that every piece of information transmitted through every website feature constitutes protected health information under HIPAA.
12. Website Accounts and Security
If you create or receive login credentials, you are responsible for maintaining their confidentiality and for activity occurring through your account to the extent permitted by law.
Notify Revive Medical Wellness promptly if you believe your credentials or account have been compromised.
You may not attempt to access another person's account or interfere with the security or operation of our systems.
13. Intellectual Property
Unless otherwise indicated, the website and its original content—including text, graphics, designs, photographs, videos, logos, trademarks, icons, layouts, and other materials—are owned by or licensed to Revive Medical Wellness and are protected by applicable intellectual-property laws.
You may access these materials for personal, noncommercial use.
You may not reproduce, modify, distribute, sell, publish, publicly display, commercially exploit, or create derivative works from protected materials without prior written permission, except where applicable law permits otherwise.
14. Testimonials, Reviews and Photographs
Patient testimonials and reviews reflect individual experiences and do not guarantee that another patient will achieve the same outcome.
Revive Medical Wellness will obtain any authorization required by applicable law before using identifiable patient photographs, videos, testimonials, or protected health information for purposes requiring such authorization.
Submitting a review through an independent public platform is subject to that platform's terms.
15. Acceptable Use
You may not use our website to engage in unlawful activity; impersonate another person; introduce malicious software; interfere with website security; attempt unauthorized access; scrape or harvest information in violation of applicable law; infringe intellectual-property rights; or harass, threaten, defame, or harm another person.
We may restrict access when reasonably necessary to protect patients, personnel, systems, or legal rights.
16. Right to Refuse or Discontinue Non-Emergency Services
To the extent permitted by applicable law and professional obligations, Revive Medical Wellness reserves the right to decline or discontinue non-emergency services when clinically appropriate or because of abusive or threatening behavior, fraud, nonpayment, repeated policy violations, breakdown of the therapeutic relationship, or other legitimate reasons.
Termination of an established clinical relationship will be handled in accordance with applicable professional and legal obligations.
17. Website Disclaimer
To the fullest extent permitted by applicable law, the website and general informational content are provided on an “AS IS” and “AS AVAILABLE” basis.
Revive Medical Wellness does not warrant that the website will always be uninterrupted, error-free, secure, or free from harmful technological components.
This disclaimer does not waive warranties or obligations that cannot lawfully be disclaimed.
18. Limitation of Liability
To the fullest extent permitted by applicable law, Revive Medical Wellness will not be liable for indirect, incidental, special, exemplary, or consequential damages arising solely from use of or inability to use the website.
Nothing in these Terms is intended to waive, release, restrict, or alter liability for professional negligence, medical malpractice, statutory rights, or other liability that cannot legally be waived or limited.
19. Indemnification
To the extent permitted by law, you agree to indemnify and hold harmless Revive Medical Wellness and its owners, officers, employees, contractors, and agents from third-party claims arising from your unlawful misuse of the website, violation of another person's rights, or material violation of these Terms.
This provision does not require a patient to indemnify Revive Medical Wellness for Revive Medical Wellness's own professional negligence or other liability that cannot lawfully be shifted to the patient.
20. Minors
The general website is not directed to children under 13 for independent use.
Medical services for minors, where offered, are subject to applicable consent requirements and clinic policies.
If we learn that personal information has been collected from a child in circumstances where collection was not permitted, we will take appropriate steps consistent with applicable law.
21. Electronic Communications
When you provide contact information or communicate electronically with Revive Medical Wellness, you consent to receiving communications reasonably necessary to administer your inquiry, appointment, account, purchase, or treatment relationship, subject to applicable law.
Marketing communications will be handled in accordance with applicable consent and opt-out requirements.
22. Changes to These Terms
Revive Medical Wellness may update these Terms periodically.
The revised Terms will display an updated effective or “Last Updated” date. Changes will apply prospectively as permitted by law.
When legally required, additional notice or consent will be obtained.
23. Governing Law
These Terms are governed by the laws of the State of Colorado, without regard to conflict-of-law principles, except where federal law or another mandatory law applies.
Any forum-selection provision should be interpreted only to the extent permitted by applicable law.
24. Severability
If any provision of these Terms is determined to be invalid or unenforceable, that provision will be enforced to the maximum extent permitted or severed as appropriate, and the remaining provisions will continue in effect.
25. No Waiver
Failure by Revive Medical Wellness to enforce a provision of these Terms on one occasion does not constitute a waiver of that provision or the right to enforce it later.
26. Entire Agreement
These Terms, together with applicable privacy notices and any separate agreements expressly incorporated into them, constitute the agreement governing the matters addressed here.
Treatment-specific informed consents, membership agreements, financial policies, prescriptions, and other separately executed patient documents remain independently applicable according to their terms.
